Residency Affidavits
Procedures for the 2020-2021 School Year
A parent/guardian seeking to register a child or children using a Residency Affidavit, should adhere to the following procedures:
Step One
Complete the Online Registration (OLR) Application AND record the OLR application number on the Residency Affidavit Form.
Contact Mrs. Thalia Carey, secretary for the HCSD H.O.P.E. Center, at 706-628-4206, ext. 2301 to request a Residency Affidavit packet.
Upon request, the packet will be provided to the requestor via email, fax, or postal service.
Step Two
Before the affidavit meeting, the following documents must be submitted for review/approval:
Homeowner/Leaseholder
- Government issued Photo Identification
- Two (2) proofs of residency
Parent/Guardian
- Government-issued Photo Identification
- One (1) proof of residency
All documents must be dated within 60 days of submission. Acceptable proofs of residency includes: Mortgage agreement, lease agreement, homestead exemption,
automobile tag/registration, voter registration, landline telephone bill, utility bill (water, electric, or gas), occupancy permit, and other good and sufficient evidence/
information requested by the district to demonstrate residency.

Step Three
Upon approval of submitted documents, Mrs. Carey will call the parent/guardian to set up a Residency Affidavit meeting.
Homeowner or leaseholder AND parent/guardian must attend the meeting.
The original, notarized Residency Affidavit Form must be presented at the meeting. * Do not submit the completed document prior to meeting.
Note: The HCSD staff does not provide notary services during this process
NOTE:
In an effort to maintain a seamless and safe process for conducting Residency Affidavit meetings, it is requested that all parties adhere to the following procedures:
Upon arrival at The H.O.P.E. Center, please park in the designated area on the side of the building.
Call the office at 706-628-4206, ext. 2301 to inform staff of your arrival.
Remain in the vehicle for additional instructions.
The Residency Affidavit meeting is limited to two participants. Only the homeowner/leaseholder and the parent/guardian will be permitted in the building to participate in the meeting.
If you are ill, please reschedule your appointment until such time symptoms have improved.
If you will be late for the appointment, it is requested that you call the office as all meetings are time sensitive and spaced to allow for the sanitization of the meeting space prior to the next appointment.
Please be advised that by completing the Residency Affidavit form, you are attesting to the statement that your family resides in Harris County and therefore your child(ren) may be granted the privilege of attending school within the Harris County School District.
